Itsuki always wears fundoshi.

If you mean panels three and four, per Obbl, "「ヨツ」is probably supposed to be 「ヨシ」which is an exclamation of excitement. 「ゲボ」is an onomatopoeia for throwing up."

Falstaff is probably wearing a piece of string. :B

I'm missing the part where Itsuki actually ate the hotdog. I know it happened after panel 3, but I'm not sure if I should read the last split one top down or bottom up.

It's called a "fundoshi" and Itsuki has worn it since the first time he appeared in the strip. It's just his Japanese style.
