The speech that Tarot makes on the second panel described the logical fallacies of this story and, to a great extent, that of the entire Saw movie series to a T!

As for why the Doc was so bad at taking the Judges pulse? I reckon that was a 'plan' worked out in the dark. "I'll fake death and thus hunt the killer if you help" sort of thing.
So the answer is finally revealed
and the culprit is all but unmasked
It's all been a game, one of fate sealed,
Justice to be done, the Judge to be tasked.
Potting colours like potting killers
He wove his way around the table,
murdering people between strip fillers
and being ironic where he was able.
What did they discuss there in the black,
the docile Doctor and the jailing Judge.
The pulse he 'took' was obviously slack
and his death seemed merely a nudge.
Two furry Indians, having no fun,
one found a gun. Then there was one.
One Furry Indian fails to be a hero
confronts the killer. Then there was zero.
